Contest Description and Duration
The “Canadian Optical Lab Survey” contest is conducted by VuePoint IDS Inc. on the web at Optik NOW. The contest will be conducted between May 15, 2018 until August 31, 2018.
Eligibility
This contest is open to any person who resides in Canada who is 18 years of age or over and works at an eye care retail location in Canada, and who completes the online survey. This contest is not open to employees, representatives and agents of the contest organizers, their subsidiaries, advertising or promotion agencies, contest prize, material or services providers, and members of their immediate families (brothers, sisters, children, father, mother), legal or common-law spouses and any other persons domiciled with these employees, agents or representatives.
How to Enter
Enter the contest by:
Electronically completing and submitting the survey available prior to August 31, 2018 at 11:59 p.m. local time Toronto (Ontario), a link to which is available on the web site optiknow.ca/contests/ (English) and optiknow.ca/fr/sondages/ (French) and/or provided through other electronic means, including email and social media posts. Limit of one entry per individual respondent.
No purchase or sale is necessary.
Prizes
The following prizes are offered:
- Five (5) prizes of a $100 each for a GIFTPASS™ from www.giftcertificates.ca will be awarded as prizes
- Total value of all prizes is $500
- All currency is Canadian $
The odds of winning the contest are determined by the number of entries received.
Each entry is eligible to receive only one prize.
Draw
The winner will be drawn within 30 days of the end of the contest period among the entrants registered . The draws will take place at the offices of VuePoint IDS Inc. Names of the winners will be published on the website page and in other VuePoint Publications, Acceptance of the prize by the entrant also provides permission to publicize the winners’ names as indicated above.
Protection of Personal Information
Personal information, i.e. information regarding an individual which allows their identification (e.g. name, business address, personal email address, telephone number, home or personal cell), are valuable. VuePoint’s policy is to manage the collection, use, confidentiality and disclosure of personal information in accordance with applicable legal requirements, including the standards set out in the Personal Information Protection and Electronic Documents Act.
Miscellaneous Conditions
To be declared a winner, the entrant selected must:
- Be reached by email and/or telephone by the contest organizers within fifteen (15) days following the draw.
- Answer a mathematical question and sign the Declaration Form and waiver of liability form (hereinafter the ‘Declaration Form’) that will be sent to him or her by the contest organizers. The Declaration Form must be duly completed and returned to the contest organizers within fifteen (15) days following its receipt by the entrant selected.
If the entrant fails to respect any of the conditions mentioned above or any other condition provided in these contest rules, the entrant selected will be disqualified and a new draw will be held in compliance with these contest rules until an entrant is selected and declared the winner.
In the four (4) weeks following receipt of the duly completed Declaration Form, the contest organizers will have the prize delivered to the winner.
The entry ballots and the Declaration Forms are subject to verification by the contest organizers. Any entry or Declaration Form that is incomplete, illegible, mutilated, fraudulent, reproduced by hand or otherwise, obtained from an unauthorized source, submitted or sent late, that doesn’t have the right answer to the mathematical question, that does not allow the entrant to be identified, will be rejected and will not entitle to an entry or to a prize, as the case may be.
Any decision made by the contest organizers regarding any aspect of this contest is final and without right of appeal, subject to any decision made by the Régie des alcools, des courses et des jeux du Québec in connection with any question falling within its jurisdiction.
The prize must be accepted as it is described in these contest rules and is non-transferable, and may not be substituted for another prize or exchanged in part or in whole for cash.
In the event that, for reasons unrelated to the winners, the contest organizers are unable to attribute the prize as described in these contest rules, the right to award a prize of similar nature and equal value or, at their sole discretion, at the cash value of the prize as indicated herein is reserved by the contest organizer.
Refusal by any person whose name is drawn at random to accept a prize as awarded, according to these contest rules, releases contest organizers from any obligation involving this prize as it applies to said person.
The right to, at sole discretion of the contest organizers, to cancel, terminate, modify or suspend this contest, in whole or in part, in the event of the occurrence of any event or human intervention that could corrupt or affect the administration, security, impartiality or the normal operation of the contest as provided in these contest rules, subject to the approval by the Régie des alcools, des courses et des jeux du Québec, if required. In any case, the contest organizers, cannot be bound to award more than one prize or to award a prize other than in compliance with these contest rules.
By participating or attempting to participate in this contest, any person releases the contest organizers, their subsidiaries, advertising and promotion agencies, employees, agents and representatives from any liability and any damages that could result from participating or attempting to participate in this contest.
By participating in this contest, the winning person authorizes the organizers and their representatives to use, if required, their name, photograph, image, declaration regarding the prize, place of residence and/or voice for advertising, with no form of compensation.
No communication or correspondence will be exchanged with the entrants (except for confirmation of entry) in this contest except with people selected for a prize.
Any dispute regarding the organization or conduct of a promotional contest may be submitted to the Régie des alcools, des courses et des jeux du Québec for a ruling. Any dispute regarding the awarding of a prize may be submitted to the Régie only for the purpose of attempting to settle it.
For the purposes of these contest rules, the entrant is the person whose name appears on the entry ballot and the prize will be awarded to that person if they are selected and declared a winner.
Either the inability or failure of the winner to execute the GIFTPASS prize through the www.giftcertificates.ca website shall not be the responsibility of the contest organizer. In such case of inability or failure the contest organizer will have no further obligations to the winner or winners.
The English version of the contest rules shall be the final applicable version in all provinces, except Quebec, for which the French version shall be applicable.
